Interesting and necessary post! Sometimes I fall prey to #1 (distraction and noice). When someone comes into my office and I'm in the middle of something I try to consciously break myself from the activity to give that person my full attention.

It is difficult to do this but I notice that people appreciate my full attention much more than when I'm writing an email while the other person is speaking.
